What is our Patient Portal?
On this web portal you may securely:

  • Schedule Doctor's Appointments (in office visits and telemedicine visits) & Lab Appointments Online 
  • Send a portal message to the phones/appointments desk to request an appointment; you'll need to call the office at 708-484-0042 to schedule a telemedicine visit.
  • Confirm appointments with your physician
  • Enter or modify personal information
  • Communicate with your physician regarding medical questions, prescription refills, referrals and lab results
  • Communicate with our office staff
  • View Lab Results, Vital Signs, and Immunization Records
  • Pay your bill (We accept Debit Cards, Visa, Mastercard and Discover
