• Schedule  Doctor &Lab Appointments Online 
  
• Send a portal message to the phones/appointments desk to request an appointment; you'll need to call the office at 708-484-0042 to schedule a telemedicine visit.
  
• Confirm appointments with your physician
• Enter or modify personal information
• Communicate with your physician regarding medical questions, prescription refills, referrals and lab results
• Communicate with our office staff
• View Lab Results, Vital Signs, and Immunization Records
• Pay your bill (We accept Debit Cards, Visa, Mastercard and Discover

DO NOT USE THE PORTAL FOR ANY URGENT MEDICAL PROBLEMS.

Covid 19: In an effort to keep everyone safe and healthy, the doctors will see well patients in the regular office, have telemedicine evaluations when appropriate, and now see sick patients by appointment in our sick only area with a separate back door entrance. For any telemedicine visit, we will bill the insurance company just like a  regular office visit.
